Registration

Upon your provider’s referral to our service, our team will contact you by phone to begin your registration process, schedule your first appointment, and answer any questions you may have prior to your first visit.

Cost & Payment

In most cases this service will be covered by your insurance provider with a co-pay. This co-pay amount will be billed to you at the end of each month. We will work with each patient to ensure cost does not limit access to the service with help from patient financial services.

Parking

Please park near and enter using Entrance 2, located off 17th Street, to visit the clinic. For your convenience, patient assistance is available at this entrance, if needed.
The Medication Management Clinic is located on the first floor of Columbus Regional Hospital next to the Emergency Department.

Personalized Patient Care

Understanding how to correctly and safely take your medicine is an extremely important part of managing your health and wellbeing.

Our goal at the Medication Management Clinic of Columbus Regional Health is to partner with you to provide the most complete and thorough care when it comes to using your medications. By providing customized education we can help you get the most benefit from your medications while minimizing side effects and complications.

Anticoagulation Services

Patients with a variety of health conditions require medicines that help prevent excess clotting of the blood. This group of medicines is commonly referred to as anticoagulants. Coumadin™ (warfarin) is one such medicine that needs routine monitoring by your health care team to make sure your blood is thinned appropriately, without experiencing unwanted side effects.

When a patient is referred for anticoagulation, they will receive one-on-one personalized care with an anticoagulation certified pharmacist. Monitoring will consist of routine blood testing and review of factors that can affect how much Coumadin™ (warfarin) each individual needs. Patients receive clear instructions on how to take their Coumadin™ (warfarin), as well as personalized education with each visit. For patients who are appropriate for home testing, our pharmacists can also provide the same exceptional level of care by phone, if requested.

Heart Failure, Hypertension, Dyslipidemia, Diabetes

This service focuses on optimizing therapeutic outcomes, preventing medication-related adverse events and providing one-on-one education. Medications related to the specific disease state requested are the focus of these services. Each appointment is approximately one hour and can be a one-time visit or, if needed, can be multiple visits based on patient needs. Upon referral, each patient will receive care from an expertly trained pharmacist, including:

  • Extensive, personalized medication education
  • Review of dosing for best impact
  • Optimization of therapy
  • Plan to maximize compliance
  • Education on how to best take the medication
  • Other patient specific suggestions

Any potential change or discontinuation of a medication is discussed with the referring provider for approval before the patient is instructed to make that change. Our pharmacists will also provide education and guidance on self-care behaviors related to the specific disease state requested, such as diet or lifestyle modifications.

Medication Therapy Management (MTM)

This service is provided to patients who are using multiple medications and experiencing problems or side effects.

When different medications are required for a patient, it is easy to become confused on what each one is used to treat. During routine annual visits, providers often find patients reporting such medication concerns. Our pharmacists will devote uninterrupted time and attention to focus on medication concerns.

After referral, the patient receives one-on-one personalized care from our MTM trained pharmacists. The patient medication list is reviewed and discussed in its entirety. This discussion could include:

  • Education on the use of a medication
  • Administration tips for medication(s)
  • Review opportunities to decrease adverse effects, or even reduce pill burden

These visits are approximately one hour and can be a one-time visit or, if needed, can be multiple visits based on patient needs. Any potential change or discontinuation of a medication is discussed with the referring provider for approval before the patient is instructed to make that change.
